Independent & Daily Living Skills
Support with learning and strengthening everyday routines and practical skills that can make daily life more manageable and independent. Examples may include meal planning and preparation, household routines, organization, planning and other everyday living skills.
Life-Skills Development
We support individuals in practising skills that contribute to confidence, decision-making and greater independence. Goals are broken into realistic steps and approached at the individual’s pace.
Community Participation
Meaningful participation based on each person’s interests, comfort level and goals—whether that involves recreation, errands, community activities, social opportunities or navigating community settings with greater confidence.
Goal Setting & Confidence Building
We work with individuals to identify what they want to learn or accomplish and support them as they build confidence through practice and experience.
Appointment & Community Assistance
Where included within an individual’s agreed support plan and Medak’s approved service scope, assistance may be available for appointments, errands and community-based activities.
Family Communication & Collaboration
When appropriate and with respect for the individual’s rights, preferences and consent, Medak values constructive communication with families and others in the person’s support network.
